Dynamic changes of ICL vault after myopic phakic lens implantation

To evaluate the early changes in the central vault after EVO Visian® ICL implantation to correct moderate to high myopia and to assess the correlation between vault changes and other relevant factors affecting vault.

The prospective study included 35 eyes of 29 females and 6 males, mean age 28.54 (22-36), mean spherical equivalent –9.47 ± 3.71 diopters (D) (range: –5.5 to –12.25 D) who underwent EVO Visian® ICL implantation (26 eyes with myopic Evo Visian®ICL, 9 eyes with myopic toric EVO Visian®ICL). Besides a complete ophthalmologic examination, the central vaulting was measured with OCT to evaluate the changes in follow-up (1 week, 1 and 3 months) and other factors affecting vault was investigated - anterior chamber depth (ACD) and horizontal white to white diameter (WTW).


The mean postoperative vault after 1 week was 495.89 ± 120.07 µm while the mean ICL vault after 1 month was 521.32 ± 155.72 µm, and the central vault at 3 months after surgery was 517.89 ± 160.07 µm. There was significant difference between the vault at 1 week and 1 and 3 month after surgery (P = 0.02, P =0.03), while no significant difference was between the vault at 1 and 3 month after surgery (P = 0.08). The central vault was positively correlated with ACD (r = 0.366, P = 0.022), and WTW (r =0.344, P = 0.034)


Our results demonstrated the dynamic changes in vault of phakic ICL in eyes in the 1 week, 1 and 3months period following ICL implantation. We concluded that preoperative ACD and WTW are contributing factors affecting the vault postoperatively. There was a trend for the phakic IOL vaults to increase from 1 week to 1 month after surgery with the vault tending to stabilize from 1 to 3 months postoperatively.
