Third and fourth generation formulas for intraocular lens power calculation after implantable collamer lens implantation in high myopia

To determine the effect of Visian Implantable Collamer Lens (ICL) implantation on biometric parameters and intraocular lens (IOL) power calculation

Methods: Setting: Institutional setting. Study population: Total of 12 patients. Observation procedure: The Zeiss IOLMaster 500 was used to measure biometry parameters and IOL power calculations before and 2 months after ICL implantation. Main outcome measures: Axial length (AL), Keratometry values (K-values) and anterior chamber depth (ACD) were measured and compared. IOL power was calculated using third (3rd) generation (SRK/T, Holladay 1, Hoffer Q) and fourth (4th) generation formulas (Haigis and Barrett Universal II) and compared.


Pre-operative and post-operative AL were comparable (27.34 ± 1.51 mm vs.27.35 ± 1.59 mm, P=0.91), same as K-values. The difference was significant between pre and post-op ACD (3.67 ± 0.46 mm vs.3.4 ± 0.56 mm, P=0.008).The average IOL power calculation showed a non-significant change using Haigis (10.04 ± 3.42 D vs.10.1 ± 3.59 D, P= 0.69); SRK/T (9.85 ± 3.41 D vs.9.94 ± 3.58 D, P=0.44); Holladay 1 (9.7 ± 3.47 D vs.9.8 ± 3.64 D, P=0.45); Hoffer Q (9.7 ± 3.4 D vs.9.85 ± 3.59, P=0.37) and Barrett Universal II (9.29 ± 3.52 D vs.9.35 ± 3.71 D, P=0.63).


ICL implantation did not affect the IOL power prediction for high myopic and myopic astigmatism patients. There is no need to use a regression formula to predict the pre-op ACD from the post-op value. In those patients whose pre-op data is not available, the post-op values can be used in the IOL power calculation formulas
