Ocular optical quality after small incision lenticule extraction (SMILE) surgery to correct myopia

To evaluate the outcomes of Small Incision Lenticule Extraction surgery (SMILE) on vision and ocular optical quality in myopic patients

This is a prospective consecutive observational case series on 24 eyes of 12 patients who underwent SMILE to correct myopia. Visual outcomes, refractive status, and corneal topography and aberrometry were noted. Ocular aberrations were assessed with the pyramidal aberrometer Osiris® (CSO, Florencia, Italia). Ocular optical quality and tear film influence on ocular optical quality were evaluated with the third-generation Optical Quality Analysis System (OQAS®, Visiometrics S L, Terrasa, Spain). Evaluations were performed preoperatively, 1 month, and 3 months after surgery - and documented.


Mean preoperative spherical equivalent (SE) was -6.43±1.85D, mean postoperative SE was -0.36±0.61D, (p<0.01). The outcomes of predictability show that 75% and 92% of eyes had a spherical equivalent within ±0.50D and ±1.00D, respectively. Mean postoperative uncorrected visual acuity (UVA) was 0.05 LogMAR. Efficacy index was 0.87, safety index was 1.02. Preoperative corneal root mean square (RMS) high order aberrations (HOA) was 0.36±0.11µm, postoperative RMS HOA was 0.72±0.25µm (p<0.01). Mean postoperative ocular Strehl Ratio was 0.182±0.052, mean modulation transfer function (MTF) cut-off point was 33.02±11.09 cycles per degree (cpd).


SMILE is an effective and safe option for the correction of myopia, and provides good post-operative ocular optical quality.
