To report the swept source anterior segment OCT (AS OCT) findings in a 37-year-old patient with iridocorneal endothelial (ICE) syndrome.

Single case report.


A 37-year-old male presented with progressive left eye (LE) visual loss. Ophthalmological examination was unremarkable in the right eye. Slit-lamp examination revealed features of ICE syndrome with high intraocular pressure in the LE. Gonioscopy showed angle closure over 180°. AS-OCT showed abnormalities of the cornea, iris and the angle. Corneal sections demonstrated hyper-reflectivity in the anterior stroma. A fine irregular hyper-reflective line with a few notches was detected underneath the endothelium. Iridocorneal angle sections demonstrated areas of synechiae associated with areas of marked angle enlargement. The study of the iris showed corectopia and attenuation of iris curvatures with hypoplasia.


AS OCT enables to assess structural modifications in ICE syndrome. The presence of endothelial hyper-reflective line supports the hypothesis of “proliferative endotheliopathy”.
