Effect of IOP pre and post KDB procedure along with cataract extraction in open-angle and angle-closure glaucoma: head-to-head, a 6 month analysis

We compared 2 sets of groups head to head following KDB ab interno trab procedure, Kahook dual blade is not new,however not many glaucoma specialists perform this procedure largely because of expert skills needed ,however if performed well , its a very handy procedure which can decrease IOP in POAG as well as PACG patients, and provides another option before trabeculectomy.

We selected 27 eyes of 24 patients at Colchester hospital university NHS Foundation trust, type of glaucoma includes 14 eyes of POAG,12 eyes of PACG and 1 angle recession glaucoma, we performed KDB procedure along with cataract extraction ( phaco ) and performed ab interno trabeculotomy by Kahook dual blade after IOL insertion , KDB was mostly performed 120 degree and few between 50-90 degree . Patients IOP was measured between 1-2 weeks, then at 3 months, then at 6 months.


Average pre-op IOP was measured 28.59, and postoperatively average IOP measured at 1-2 weeks was 16.7, at 3 months IOP was 16.25 and at 6 months 13 mmHg, with 4 patients did not turn up for 6 months follow-up and 3 patients went on to have antiglaucoma medications and 2 needed Xen gel implant later. POAG group patient had 19.2 mmHg IOP pre-operatively and at 6 months 13 mmHg was noted. PACG group had 24.5 mmHg IOP and at 6 months and at 6 months 12.7


We found that PACG group surprisingly did well after KDB and cataract surgery together, and POAG group also had very nice IOP control without any medication. A larger trial needed in order to include more number of patients and long-term follow-up needed in order to generalize the outcome.
