Comparative results evaluation of IOL exchange and IOL reposition with iris suture fixation in cases of spontaneous in-the-bag IOL dislocation

Abstract Details


The aim of this study was to compare the iris suturing technique and IOL exchange surgery for late spontaneous in-the-bag IOL dislocation into the anterior vitreous cavity.

Twenty cases of late spontaneous in-the-bag IOL dislocation were studied. Ultrasound biomicroscopy data confirmed the weakening or breaking of a part of the zonules in several segments. In 10 cases IOL reposition with suturing to the iris was performed, in the other 10 cases IOL was removed and irido-vitreal IOL was implanted. Both techniques were performed under topical anesthesia. Visual acuity, refraction, IOP, endothelial cell density, pupil configuration before and after surgery were evaluated.


The IOL reposition with suturing to the iris is technically simple and microinvasive method, that restores the proper position of the IOL and visual functions of the eye. The complications of the first technique included non-significant intraoperative iris bleeding (in 1 case), postoperative IOP elevation (in 4 cases), managed by hypotensive drops. After the exchange surgery no intraoperative complications occured, in 3 cases IOP elevation was detected postoperatively, managed by hypotensive drops. IOL centeration was achieved in both groups.


Both methods allow to restore visual functions. The IOL reposition with suturing to the iris is a simple technique, that is cost-effective and reduces rehabilitation period. According to pupillometry data suturing technique does not change the iris diaphragm function.
