Prechop without prechoppers outside the capsular bag in small capsulorhexis

To demonstrate that prechop techniques outside the capsular bag are possible even with a smal size (5.5 mm) capsulorhexis.

100 phacoemulsification cases were operated by one surgeon using 0.7 mm biaxial technique and performing prechop of the nucleus, without prechoppers, outside the capsular bag after performing a small manual capsulorhexis (5.5 mm). Surgical outcomes (final best corrected visual acuity, rate of posterior capsule rupture, rate of posterior capsule opacification, amount of needed instrumentation and incision trauma) were measured and compared with files of 100 eyes operated with phacoemulsification with prechop inside the capsular bag, with the use of prechoppers.


No differences were found between both techniques in final best corrected visual acuity and rate of posterior capsular opacification. Significative differences were found in rate of posterior capsular rupture, amount of needed instrumentation and trauma to the incision.


Prechop outside the capsular bag, avoiding capsular trauma, is possible after performing a 5.5 mm manual capsulorhexis, It allows to mantain the intraocular lens rim inside the capsulorhexis edge, decreasing the rate of posterior capsular opacification, but also decreasing the rate of capsular complications of performing prechop inside the capsular bag.
