Abstract Details


To report the visual and refractive outcomes following implantation of a small aperture intraocular lens (IC – 8, Acufocus) in subjects undergoing cataract surgery in eyes with previous radial keratotomy (RK).

Interventional case series. 8 eyes of seven patients with visually significant cataract and previous history of RK for myopia were included. All underwent either unilateral or bilateral implantation (two patients had bilateral implantation) of the small aperture IC 8 IOL during cataract surgery. In subjects who had unilateral implantation, the dominant eye was targeted for emmetropia with a monofocal IOL while the small aperture IOL was implanted in the non dominant eye aiming of mild myopia (-0.75 D). The ASCRS post RK calculator was used to IOL power calculations.


There were no intraoperative complications. The mean uncorrected visual acuity improved from 1.0 (log mar) preoperatively to 0.2 post operatively. The uncorrected intermediate vision improved from 0.8 preoperatively to 0.2 and uncorrected near vision in the eyes with the small aperture IOL improved from 0.7 preoperatively to 0.2 post operatively


The small aperture IOL seems to provide good uncorrected distance, intermediate and near vision following cataract surgery in eyes with previous RK and should be considered as a safe and viable IOL option for this subset of patients.
