Balanced salt solution (BSS) assisted IOL implantation: intraocular pressure and endothelial study

To evaluate the effect of BSS assisted IOL implantation on postoperative IOP and endothelial cells and to compare BSS assisted IOL implantation with standart viscoelastic asssisted IOL implantation during phacoemilsification.


A prospective, randomized clinical study


40 eyes of 30 patients (10 women, 20 men) with cataract and undergoing phacoemilsification surgery with BSS assisted (22 eyes) or viscoelastic assisted IOL (18 eyes) implantation were evaluated. BSS assisted IOL implantation was performed with the aid of irrigation cannula and BSS without using viscoelastic. Viscoelastic assisted IOL implantation was performed standardly that IOL was implantated with the aid of cohesive viscoelastic. IOP and endothelial changes after phacoemulsification surgeries and the surgical time were noted and compared. And also, the eyes were evaluated in terms of cataract density preoperatively and phaco parameters peroperatively.


The mean age was 65.5±8.5 years in BSS assisted group and 66.9±11.4 in viscoelastic assisted group. The mean preoperative IOP, postoperative IOP at 1. day, 1. week and 1 . month was 14.0±2.0, 15.8±3.9, 13.7±1.9 and 13.0±2.2 respectively for BSS assisted and 14.6±1.9, 19.7±4.4, 13.6±1.6 and 13.2±2.0 respectively for viscoelastic assisted IOL implantation. IOP increase was significantly higher at first day in viscoelastic assisted IOL implantation group (p=0.008). The surgical time was 12.5±2.8 minute in BSS assisted group and 14.2±2.5 in viscoelastic assisted group (p=0.04). The endothelial changes were not statistically significant between both group (p=0.134).


IOL implantation with BSS assisted during phaemulsification accelerated cataract surgery. This method caused minimal IOP increase and may be useful particulary for glaucoma patients.
