To report and study a DSAEK complication associated with the use of hydrophilic acrylic IOLs

A retrospective study of 5 cases from our center presented with anterior surface IOL opacification following an endothelial keratoplasty


In all 5 cases, the anterior surface of the hydrophilic acrylic IOL was opacified. All 5 had underwent a routine DSAEK in the past 12 to 28 months. At the time of the DSAEK procedure, all the patients were already pseudophakic having a hydrophilic acrylic IOL implant used. In all the procedures air was used for bubbling. In none of them rebubbling was needed. Opacification was clinical significant in all patients with BCVA from 2/10 to 6/10


IOL opacification following DSAEK seems to be a newly described late complication associated with the air used during the keratoplasty procedure. As IOL opacification is highly connected with the use of hydrophilic acrylic IOLs, it is important to avoid the use of hydrophilic acrylic IOLs in patients with increased risk factors for having an endothelial keratoplasty in the future FINANCIAL DISCLOUSRE: NONE
