Visual outcomes and flap thickness in myopic femto-LASIK: a comparison of three devices

To compare visual outcomes and flap thickness during a 3 month follow up using different femtosecond platforms (FS) to correct myopia. We compared the iFS 150, FS200 and LenSx for flap shaping. The same excimer platform (Allegretto®) was used for the corneal ablations.

This is an observational, prospective cohort study. 36 consecutive eyes were included in each laser group. Uncorrected distance visual acuity (UDVA) and high definition corneal OCT (Heidelberg Spectralis®) were made in each post-op visit (at 1 day, 1 week, 1 and 3 months). OCT images where manually measured with the caliper provided in the software of the device. The same, masked observer, measured the flap thickness (FT) in the center of the flap and 500 microns nasally and temporally. The average was considered the central flap thickness (CFT).


First day post-op, statistically significant differences were found in UCVA, (1.050.19 vs 1.070,14 vs 0.940.21) (p=0.004) in the iFS, LenSx and FS200, respectively. The iFS150 had thinner and more regular CFT (107.796,93 vs 126.5111.36 vs 127.9412.09) (p=0.0001). After 1 week, the iFS and LenSx obtained better UCVA than the FS200 (1.08 0.16 vs 1.09 0.16 vs 0.930.18) (p=0.0001), and the CFT was (106.845.91 vs 125.729.34 vs 127.4311.96) (p=0.0001). At 1 and 3 months, UCVA were similar in all groups, while the iFS still provided statistically significant better CFT (3 months: 116.094.70 vs 126.508.44 vs 131.787.21) (p=0.0001)


Differences in UCVA are present in early post-op after FS-LASIK, with all platforms showing comparable results at 1 and 3 months post-op. iFS150 and LenSx obtained earlier VA recovery. The iFS150 flaps were thinner and more homogeneous, showing a significant increase in CFT in the follow up. The other groups had thicker flaps since the beginning. Some transient reason must be inducing an increase in the CFT in the LenSx and the FS200 eyes.
