Efficacy and predictability of LASIK for the correction of low myopia and compound myopic astigmatism ≤ 0.75 diopter

To examine the refractive and visual outcome of wavefront-optimized laser in situ keratomileusis (LASIK) in eyes with low myopia and compound myopic astigmatism ? 0.75 diopter (D).

This study comprised 163 eyes of 163 consecutive myopic patients (76 male, 87 female; mean age at surgery 40.3 ±10.6 years) with a completed 3 month follow-up, preoperative refractive cylinder ? 0.75 D and manifest sphere between -0.25 D and -2.75 D. Three subgroups were formed with respect to preoperative manifest refractive cylinder magnitude (0.25, 0.50, and 0.75 D). Manifest refraction, uncorrected and corrected visual acuity were assessed pre- and postoperatively thereby focusing on the role of the preoperative cylinder axis. The astigmatic changes achieved were determined by using the Alpins vector analysis.


By 4 months (120.5 ±27.3 days) follow-up, we obtained a mean uncorrected distant visual acuity (UDVA) of 0.08±0.13 logMAR and a mean manifest refraction spherical equivalent (MRSE) of -0.05 ±0.57 D. There was no statistically significant difference regarding efficacy and safety between the preoperative cylinder groups. Astigmatic overcorrection for preoperative cylinder ? 0.50 D was suggested by the correction index, the magnitude of error, the index of success, and the flattening index. For different preoperative cylinder axis, we did not obtain any statistically significant differences in outcome.


Although mean UDVA and mean MRSE obtained by 4 month follow-up are appropriate, low myopic eyes with preoperative cylinder ? 0.50 D were significantly overcorrected. Accordingly, we are cautious in recommending full astigmatic correction for eyes with low myopia and manifest cylinder ? 0.50 D.
