Deep anterior lamellar keratoplasty as an alternative to penetrating keratoplasty: case control study

The aim of this study is to consider thesis that innovative technique of deep anterior lamellar keratoplasty DALK using Anwar’s technique gives better postoperative outcomes when compared to penetrating keratoplasty, reduces postoperative recuperation period, reduce amount of intra and postoperative complications as well as reduces postoperative endothelial cell lost.


: From April 2008 till February 2012 in Ophthalmology Department of District Railway Hospital in Katowice, Poland.


The study includes 90 patients (90 eyes). Study group consisted of 50 patients (50 eyes), 22 women and 28 men suffering from keratoconus, who underwent deep anterior lamellar keratoplasty using Anwar’s technique. The comparative group consisted of 40 patients (40 eyes), 16 women and 24 men, suffering from keratoconus who underwent penetrating keratoplasty.


Twelve months after surgery there was no statistically significant difference in BCVA between both groups. Mean postoperative astigmatism in study group equaled 2.48 and 1.89 in comparative one. Twelve months after surgery mean postoperative astigmatism was statistically significant lower in comparative group than in study one. CCT equaled 451.5 µm in study group and 525.47 µm in the comparative one. Mean CCT values in study group were statistically significant lower than in comparative one. Mean IOP using GAT equals 12.8 mmHg in the study group and 14.65 mmHg in comparative one. Mean IOP using IC equaled 12.3 mmHg and 14.02 mmHg in the comparative one. Mean IOP values in study group were statistically significant lower than in comparative one. Endothelial cell lost (ECL) in the study group, twelve months after surgery, equaled 7.49% and 43.21% in the comparative.


Anterior Lamellar keratoplasty using Anwar’s technique is more effective and secure surgical technique when compared to penetrating keratoplasty.
