Pachymetry, equivalent keratometric readings with zonal changes and pupil diameter that affect intraocular lens calculation after laser refractive surgery

To compare pre- and post myopic laser refract?ve surgery keratometric and pachymetr?c measurements performed with Scheimpflug camera imaging with the values obtained using the clinical history method and simulated keratometry.

The Intralase femtosecond laser (AMO Inc, Irvine, CA) was used to create the flap in all eyes that underwent LASIK. All eyes underwent wavefront-guided LASIK with MEL 80 excimer laser (Carl Zeiss, Jena, Germany) for the correction of myopia or myopic astigmatism. 62 eyes of 31 patients were included in the study. The minimum required follow-up was 6 months after LASIK. Each eye was evaluated by Scheimpflug camera imaging (Pentacam, Wetzlar, Germany) before and after surgery. The following values were analyzed and compared with those obtained with the clinical history method: mean simulated keratometry (K), mean true net power, corneal pachymetry, equivalent K reading shown in the Holladay report, and pupil diameter.


The mean K and corneal thickness decreased significantly after LASIK (p<0.05). The mean sim K given by Pentacam was higher than the mean clinical history method value (p<0.05). The mean true net power was significantly lower than the value obtained with the clinical history method (p<0.05). Among different equivalent K readings under 1.0mm, 2.0mm, 3.00mm and 4.5 mm zones, the mean equivalent K readings at 1.0 and 2.0 mm was closest to the benchmark value.. However, there was a high variability. There was a correlation with postoperative pupil diameter and equivalent k reading diameter.


The mean equivalent K readings at 1.0 and 2.0 mm was closest to the benchmark value. Although with high variability postoperative pupil diameter was correlated with equivalent k reading diameter.
