Improve the Accuracy of alignment and centering toric pseudophakic iol impantation with a new surgery guidance platform for toric IOL implantation (Surgery Guidance SMI)

. 40 eyes of 40 patient were undergoing a cataract extraction with implantation of a toric pseudophakic IOL (Acrysof toric SN60T3-T9) with SMI platform. 0/40 eyes no preoperative corneal axis marker. Preoperatively, patients had a complete ophthalmic evaluation: including manifest refraction, slitlamp examination, fundoscopy, applanation tonometry, optical biometry (IOLMaster Carl Zeiss), corneal topography (Orbscan Bausch Lomb), Surgery Guidance SMI Automated keratometry.


After 6 months cataract surgery the postoperative refractive astigmatism was significantly reduction. Six months postoperatively, 0/40 (0%) IOL rotation, tilt or decentration, no macular oedema, no retinal detachment.


Toric pseudophakic IOL implantation is a viable and highly predictable method of correcting the corneal astigmatism. Careful selection of the patient, accurate keratometry and precise alignment of the cylindrical axes are some of the factors to be considered for a superior outcome. The system SMI help the surgeon in improving the accuracy of cataract surgery and implant the toric pseudophachic IOL with regard to the location of the main incision, and especially in alignment and centering of toric IOL, so as to reduce the "unexpected" factors.
