Macular thickness changes after refractive lens exchange

To evaluate morphological macular changes after refractive lens exchange with multifocal intraocular lens implantation by means of optical coherence tomography (OCT), and to assess the influence of age, surgical parameters and biometric parameters on the results.

In a prospective study, 33 patients who underwent refractive lens exchange (RLE) with multifocal intraocular lens (MF IOL) implantation were examined. Phacoemulsification and MF IOL implantation was performed in both eyes consecutively, and 33 randomly selected eyes were included in the study. OCT examination was performed preoperatively, at 1 month and 3 months postoperatively. Foveal thickness (FT), inner macular thickness (IMT), and macular volume (MV) were analyzed. At each visit, the visual acuity (distance, intermediate and near visual acuity) test and slit-lamp examination was performed as well. Correlation of surgical parameters- Cumulative Dissipated Energy (CDE), axial length (AL) and anterior chamber depth (ACD) with macular thickness was evaluated as well. Acquired data were assessed by means of a statistical analysis using non-parametric tests and Pearson's correlation test


Mean age of study group was 57 years (min.49- max 69). The mean central foveal retinal thickness was preoperatively 208.4 ± 25.9?m, postoperatively at 1st month, 215.2 ±24.0 ?m (p=0,12), at 3rd month, 213.5 ±29.4 ?m (p =0,25). Mean preoperative IMT was 255.0 ±31.3?m, postoperatively at 1month 266.1± 29.5?m (p=0,08), at 3rd month 260.3±34?m (p=0,21),respectively. Preoperative MV 6,75mm³ increased at 1 month to 6,84mm³, and decreased to 6,81 mm³ at 3rd month. However, neither the changes in macular volume, nor in retinal thickness was statistically significant. A correlation between macular thickness, visual acuity, selected surgical and biometric parameters was not found.


After RLE with MF IOL implantation, a mild increase of foveal thickness without impact on visual acuity could be observed. Age, surgical and biometric parameters did not correlate with the degree of macular thickening. FINANCIAL DISCLOSURE?: No
