To evaluate Trans-Epithelial ablation in patients with post penetrating Keratoplasty Astigmatism.

Incidence of Keratoconus is very high in East Africa. We have been doing PKP for nearly 30 years and post Keratoplasty Astigmatism is a real challenge. In the past, we have tried incisional penetrating keratotomy like; opposite clear cornea (OCCI) incision, the Lasik and Lasek. But now, we are doing Trans-epithelial corneal regularization on the IVIS platform with special software. All the procedures were done under topical anaesthesia and it was based on Tomography regularization.


26 patients were enrolled and Trans-epithelial corneal regularization was done on Tomography based. Maximum ablation was done about 60-65 microns. Pre-Op and Post-Op Pachymetry, refraction, K-reading, Tomography and high order ablations were noted. Mitomycin .02% was used routinely.


All patients showed marked improvement of Astigmatism and Corneal regularization. On average there was an Improvement of about 5 diopter of astigmatism. The patientÂ’s refraction can be refracted at procedure. We can prescribe glasses or hybrid contact lenses. There was no sub-epithelial corneal haze and in our opinion this is a very good option relatively non-invasive option for post PKP ablations. FINANCIAL DISCLOSURE?: No
