Association of biometric factors of hyperopic eye with results of lens surgery

To evaluate anterior segment biometric factors of hyperopic eyes associated with the degree of angle widening, intraocular pressure and induced myopia reduction after lens surgery.


Ophthalmic clinic " OKOMED" , Moscow, Russia.


Clinical study included 41 eyes of 29 patients aged 35-78 years (mean age 49.8±10.2). Refraction of 23 patients was Hyperopia from +1.25 to +10.0 D, Induced Myopia of 10 patients was from -0.5 to -10.0 D, 12 patients had glaucoma with IOP (mean 31.0±2.5 mm Hg), 2 eyes lost vision from glaucoma. All eyes were operated by Phacoemulsification with Acrylic Lens implantation: Clear Lens Exchange for 23 eyes, extraction of inicial cataract with high vision acuity for 18 eyes, 3 eyes had preliminary iridectomy, 5 eyes had glaucoma and cataract surgery together. Axial eye length, lens thickness obtained by Echobiometry. Anterior segment biometric factors obtained by Optical Coherence Tomography (OPTOVUE) were compared preoperatively and 3 months postoperatively. Measurements included anterior chamber angle(ACA), anterior chamber deep (ACD), the distance iris-lens (DIL) and distance iris-cornea (DIC) in 500µm from scleral spur.


Mean axial eye length was 20.9±1.17 mm (19.66 to 22.9 mm), lens thickness was 4.57±0.23 mm (4.1 to 5.5 mm). ACA (mean 9.54±1.38°,from 7.0° to 18.0°), ACD (mean 2.31±0.37 mm, from 1.8 to 2.62 mm), DIL (mean 0.18±0.03mm, from 0.06 to 0.22 mm), DIC (mean 0.26±0.12 mm); values were significantly smaler in hyperopic eyes, especially with glaucoma. After operation we received ACD (mean 3,42±0.45 mm), ACA (mean 24.3±5.4°), DIL (mean 0.16±0.10 mm), DIC (mean 0.44±0.10 mm). IOP was 18.6± 3.6 mm Hg. Optical power of IOL was from 20.5 to 35.0 D. Postoperative refraction was ±1.0 D in 78.4%, ±2.0 D –in 88%. 48.7% eyes had UDVA 0.8-1.0.


Irregular anterior segment biometric parameters, induced myopia, glaucoma are risk factors of hyperopic eye. Anterior segment reconstruction of hyperopic eyes by clear lens exchange or cataract surgery is efficacy procedure to intraocular preassure decrease, correction hyperopia and to get high visual acuity.
