Ultrathin manually dissected DSEK (UTMDSEK)

Our aim is to describe our technique for the standardised creation of the ultrathin donor corneal lenticules. The procedure was designed in order to provide better visual outcomes without increased risk of complications of further stress to the endothelial cells

All donor corneas were supplied by NHS BT Services. Our technique involved pre-soaking the donor cornea in balanced salt solution for 30 min prior to lamellar dissection.


In 10 consecutive corneas dissected using this technique, mean graft thicknesses measured less than 100 µm at one month (mean thickness 97.4 µm, range 56-143 µm, SD 35.403 µm). Mean best corrected visual acuity (BCVA) in the five patients with visual potential was 0.28 (range 0.18-0.42) logMAR.


Our technique UTMDSEK demonstrated that consistently thin endothelial donor corneal grafts could be produced with minimal financial investment and no burden on the duration of surgery. FINANCIAL DISCLOSURE?: No
