Abstract Details


To evaluate the use and performance of an eye-tracking navigation system in the alignment of toric IOLs in refractive cataract surgery.


University Eye Clinic, Maastricht University Medical Center, Maastricht, The Netherlands.


An eye-tracking navigation system (Verion Image Guided system, Alcon Inc, Fort Worth, USA) which matches limbal blood vessels and iris features was used to perform preoperative keratometry and cyclorotation registration. A surgical plan was developed using customised planning software and a femtosecond laser-assisted cataract surgery approach. The microscope heads-up display was used to align the toric IOL. Image data sets of 67 manually marked eyes were compared to 26 eyes using the Verion Image Guided system. Postoperative misalignment was measured using a slit lamp. Keratometry, refraction, and visual acuity data were entered into a database for outcomes analysis.


The Verion reference unit was able to achieve preoperative measurements of keratometry and peroperative cyclorotation registration in all eyes. The overall error of toric misalignment was 4.1° ± 4.7° in the manually marked group vs. 2.4° ± 2.2° in the Verion group.


The Verion Image Guided system is a useful tool in preoperative planning and peroperative toric IOL alignment in cataract surgery of eyes with corneal astigmatism. The integrated, closed-loop process allows efficient planning and accurate execution of refractive cataract surgery. The preliminary results of this new marker-free surgery guidance system appear to be more accurate as compared to conventional manual marking techniques.
