Abstract Details


To evaluate a novel intraocular lens power calculation technique using continuous intra-operative optical coherence tomography (OCT) scans of the aphakic eye.

This prospective study included patients who were scheduled for cataract surgery and had no co-morbidities. Pre-operatively, optical biometry (Lenstar LS 900, Haag-Streit AG and IOL-Master 500 Carl Zeiss Meditec AG, Germany) was used to measure axial eye length, anterior chamber depth and lens thickness. A prototype operating microscope with an integrated continuous OCT device (Visante attached to OPMI VISU 200, Carl Zeiss Meditec AG), was used to measure the anterior and posterior lens capsule position at different time points during cataract surgery. In all cases, a capsular tension ring (Morcher GmbH, Germany) was used to tauten the lens capsule. One hour, 1 day and 3 months postoperatively, the ACD-measurements were repeated using optical biometry. Furthermore, autorefraction and subjective refraction were performed at the last follow-up.


In total, 170 eyes of 170 patients were included. Preliminary results showed an excellent feasibility of the continuous intra-operative OCT device and successful scans were performed in 96% of all cases. The novel intra-operative approach showed a significantly lower dependency on axial eye length compared to conventional IOL power calculation formulae (Holladay I: r2=-0.42, HofferQ: r2=-0.5, SRK/T: r2=-0.34, Haigis: r2=-0.28 and ACDintraOP: r2<0.001).


The novel intra-operative approach showed to result in a significantly lower axial eye length dependency compared to conventional IOL power calculation formulae. Therefore, especially short eyes would benefit from this technique.
