Visual acuity and optical density after myopic FemtoLASIK: a comparison of three devices

To describe the visual outcomes and the optical density (OD) of the flap during a 3 month follow up using different femtosecond laser platforms to perform the femto-LASIK for myopia correction. We compared the iFS150, FS200 and the LenSx. The same excimer platform (Allegretto) was used for the corneal ablations

Observational, prospective cohort study. We evaluated 36 consecutive eyes in each group, who underwent uneventful FS-LASIK surgery for myopia correction. Visual outcomes and corneal OCT were made in each visit (at 1 day, 1 week, 1 and 3 months post-op). The Central Flap Thickness (CFT) was measured with the caliper provided in the software. Using the ImageJ software, two regions were defined in the stroma of the flap and in the stromal bed, to obtain a measure of the optical density (OD) in gray scale units (GSU).


First day post-op, significant differences were found in the UCVA, (1.050.19 vs 1.070.14 vs 0.940.21) (p=0.004) iFS150, LenSx and FS200 respectively. The OD of the flap stroma was lower in the iFS150 eyes (134.5326.55 vs 158.9118.61 vs 149.79 27.98) (p=0.0002). After 1 week, iFS150 and LenSx still provided better UCVAs (1.08 0.16 vs 1.09 0.16 vs 0.930.18) (p=0.0001), while the OD was still lower in iFS150 (143.8826.02 vs 160.0920.99 vs 147.5732.64) (p=0.04). At 1 and 3 months post-op, UCVAs were not statistically different. The OD was no longer different at 3 months post-op.


Statistically significant differences are found in UCVA in the early post-op between devices, although the results were similar in the long term. The iFS150 and LenSx obtain better UCVA than the FS200 in the early pos-op. The OD of the flap stroma is initially lower in the iFS, showing an increase in time. On the other hand, LenSx and FS200 have flaps with higher OD in the early post-op, that decreases during the follow up.
