Ahmed valve implantation at preterminal stages of repeatedly operated uncompensated neovascular glaucoma with diagenesis of anterior chamber angle

Abstract Details

Purpose:

To analyze the modification technique of Ahmed valve implantation and to estimate the possibility of its installation in the absence of effect from antiglaucomatous invasions at preterminal stages of repeatedly operated uncompensated neovascular glaucoma with diagenesis of anterior chamber angle.

Setting:

Sv. Fyodorov Eye Microsurgery Complex, Krasnodar Branch, Russia

Methods:

The clinical study was conducted in 23 patients, 22 �â�€�“ suffer from insulin-dependent diabetes mellitus with expressed rubeosis of the iris, corneal oedema of the 2nd degree with prolonged decompensation of IOP more than 35 mmHg, visometry not more than 0.05. The first group of 12 eyes �â�€�“ Ahmed valve implantation using modified technique: instantaneous puncture with complete needle of the scleral membrane with the following capillary valve insertion through formed scleral canal into anterior chamber, the second - 11 eyes with deep sclerectomy conduction by standard technique.

Results:

In 3 months in the first group IOP in 2 patients-16mmHg, 5 patients-18mmHg , 2 patients-20mmHg, 1 patient-21mmHg, 2 patients-22 mmHg, visometry 0.05-1.0 mmHg. In the second group IOP in 3 patients-16mmHg, 1 patient-17mmHg, 2 patients-22mmHg, 2 patients-31mmHg, 3 patients-35mmHg, IOP decompensation-in 5 patients (45%), visometry 0.02-0.9.

Conclusions:

Using modified technique of Ahmed valve implantation at preterminal stages of repeatedly operated uncompensated neovascular glaucoma, the operation is performed less traumatically, that reduces the possibility of hemorrhage development in the anterior chamber, it allows to lower and stabilize IOP.
