The effects of topical 0.45% ketorolac tromethamine on intraoperative miosis and prostaglandin E2 release during femtosecond laser-assisted cataract surgery

Abstract Details

Purpose:

Efficacy of preoperative topical nonsteroidal anti-inflammatory drug (NSAID) is an undetermined issue on intraoperative miosis during femtosecond laser-assisted cataract surgery (FLACS). We aimed to determine the effects of topical 0.45% ketorolac tromethamine on intraoperative miosis and prostaglandin E2 (PGE2) release during FLACS.

Setting:

Seoul St. Mary�â�€�™s Hospital, Seoul, Korea

Methods:

One group of patients underwent conventional cataract surgery (CCS) without topical NSAIDs; FLACS with preoperative topical 0.45% ketorolac tromethamine (FLACS()); or FLACS without topical NSAIDs (FLACS(-)). To measure the aqueous humor PGE2 concentration, a 100-�Â�µL aqueous humor sample was collected from the anterior chamber after femtosecond laser pretreatments. PGE2 concentrations were measured with an enzyme immunoassay.

Results:

0.45% ketorolac tromethamine led to a significant reduction in intraoperative miosis in the FLACS() compared with the FLACS(-) group (p < 0.001). Absolute and relative reductions in pupil area in the FLACS() were significantly lower than those in the FLACS(-) group (p = 0.019 and 0.007, respectively). The mean aqueous humor PGE2 concentrations were 893.60 �Â�± 843.10 pg/mL in the CCS group, 1911.43 �Â�± 1178.63 pg/mL in the FLACS(-) group, and 743.63 �Â�± 927.46 pg/mL in the FLACS() group (p < 0.001 CCS compared to FLACS(-); p < 0.001 FLACS() compared to FLACS(-); p > 0.05 CCS compared to FLACS()).

Conclusions:

Preoperative topical 0.45% ketorolac tromethamine reduced miosis induced by femtosecond laser pretreatment, and inhibited aqueous humor PGE2 elevation.
