The advantages of femtosecond laser-assisted anterior capsulotomy in peculiar cases

Abstract Details


To present three peculiar cases who underwent cataract surgery at which the anterior capsulotomy was performed using the LenSx femtosecond laser.

Three cases were selected.The first pacient presents at RE a posttraumatic cataract with a wrinkled anterior capsule.The second case presents at LE intumescent cataract associated with stage II keratoconus.Clear-lens exchange was chosen for a high mixed astigmatism in the third case.We performed femtosecond-laser assisted 4,9 mm anterior capsulotomy using LenSx.The nucleus and the cortex of the lens were removed using Infinity or Centurion Vision System.In the first case we implanted a monofocal IOL.In the second case the implanted IOL was a monofocal toric and the third pacient underwent refractive clear-lens exchange with implantation of a multifocal toric IOL.


In the first case the anterior capsulotomy presented microadhesions in the inferior half, but the capsule was easily detached using Utrata forceps. The second case had incomplete capsulotomy spanning two clock hours in the superior half, but also being easily detached using Utrata forceps. The third case had free-floating circular capsulotomy.


The size, shape and centration of the capsulorrhexis is variable depending on the type of cataract. Our results show that femtosecond laser-assisted anterior capsulotomy in posttraumatic and intumescent cataract is accurate, precise, safe and has higher reliability than manual capsulorrhexis. A well-performed anterior capsulotomy improves the steps of white or intumescent cataract extraction and reduces complications. In cases with premium IOL implants the femto-capsulorrhexis gives accurate refractive outcomes and is predictably sized and centred.
