To report results of graft thickness and rate of complications with a novel technique for Thin Manual Descemet / non Descemet Stripping Endothelial Keratoplasty donor lenticle dissection (TM (n)DSEK).

A variation to the standard TM DSEK dissection technique was introduced. Donor material was soaked in BSS for 30 min following usual protocol. Donor cornea was mounted in an artificial anterior chamber (AAC) and dissected manually. AAC was filled with air instead of fluid. A reflection between endothelium and air bubble served as a landmark for the depth of the dissection, hence pachymetry is not required. 21 donor corneas were dissected by a novel TM DSEK surgeon. Graft thickness, time of donor dissection, and rates of perforation of donor material, graft detachment, upside down grafts and primary failure were analysed.


Results were comparable to other DSEK and TM DSEK techniques despite of all the cases being performed by a novel TM DSEK surgeon.


This may reflect greater safety with this technique especially in the hands of a novel surgeon. Donor dissection time was lower than that of standard TM DSEK donor dissection technique as less steps are required.
