Phaco rotationless technique for weak zonules

Abstract Details

Purpose:

To describe a technique for nucleus removal without rotating crystalline lens : �â�€�œwedge�â�€� technique.

Setting:

Multidisciplinary Science and Tecnology Complex Eye Microsurgery named after Academician S.N.Fyodorov Federal State Institution Novosibirsk Branch

Methods:

Between November 2011 and Decemberl 2016 438 eyes with PEX were treated by phacoemulsification without nuclear rotation using �â�€�œwedge�â�€� technique. After capsulotomy and hydrodissection left paracentral oblique groove (80-90% depth) was made. Nucleus was divided by phaco tip and chopper. In right heminucleus a lateral deep groove was performed such way to create a big asymmetric wedge including central part of nucleus. This wedge was removed without manipulation and rotation. The remaining two peripheral segments were easily gripped and pulled into pupil area for safe phacoemulsification.

Results:

In all cases this technique allowed for safe removing of largest part of the nucleus. Irrigation flows made it easily to move the remaining segments into central area.

Conclusions:

�â�€�œWedge�â�€� technique is useful for cases with weak zonules.
