Cataract surgery with implantation of multifocal intraocular lens in patients under 40 years old

Abstract Details

Purpose:

To evaluate refractive and visual outcomes and patients satisfaction after cataract surgery with implantation of multifocal intraocular lens (IOL) in patients under 40 years old.

Setting:

Ophthalmology department of a tertiary referral center in Oporto, Portugal �â�€�“ Centro Hospitalar Universit�Ã�¡rio do Porto.

Methods:

Retrospective study of 15 eyes of 9 patients (age between 29-40 years old) who were submitted to cataract surgery with multifocal lens implantation. Uncorrect distance visual acuity (UCDVA - decimal scale), best corrected distance visual acuity (BCDVA), near visual acuity (Jaeger scale) and manifest refraction were evaluated preoperatively, at 1 and 3 months postoperatively and at the end of follow-up. Dependency of spectacles, patient satisfaction after surgery, complications and surgical reinterventions were also analysed. The minimum follow-up was 6 months.

Results:

The main reason for cataract surgery was posterior subcapsular cataracts (n= 6 eyes). Three different models of multifocal IOL were used. Mean preoperative BCDVA was 0,4 and improved to 0,9 three months after surgery (p<0,001). There was a significant improvement in postoperative refractive status (p=0,05 for sphere, p=0,12 for cylinder). Only 1 patient was dependent on spectacles for distance acuity. BCVDA remained stable in 88% of the eyes throughout follow-up. Near visual acuity after surgery was J1 in 33% and J1,5 in 67% of eyes without spectacles. 2 eyes developed posterior capsular opacification and were submitted to laser capsulotomy.

Conclusions:

The visual outcomes achieved with multifocal IOL implantation were good and remain stable during follow-up. All patients were very satisfied after surgery. Cataract surgery with multifocal IOL implantation is a valid and valuable option to restore distance and near visual acuity in patients under 40 years old.
