Use of capsular tension ring in the surgery of age-related cataracts with damage to the Zinn ligaments

Evaluate the effectiveness of the capsular tenses ring in the surgery of lens subluxation of various degrees.

There were 30 eyes with age-related cataract, aged 40-84 with various degrees of sublaxation. In the preoperative period, all patients underwent ultrasonic biomicroscopy (USB). Patients visual acuity on admission was 0.05 in 15 eyes (50%), 0.05-0.1 in 9 eyes (30%), 0.1-0.3 in 6 eyes (20%). Depending on the opacity of the lens, proportion of immature cataract was in 16.7%, mature cataract was in 66.6%, hypermature cataract was in 16.7%. State of the Zinn ligaments in USB was as follows: ligament lysed in one quadrant was in 33.3%, in two quadrants was in 60%, more than two quadrants was in 6.7%.


Apparently that state of Zinn ligament requires use of intracapsular ring during phacoemulsification of cataracts. All patients for stabilizing capsular bag used retractors, fixing the edge of capsulorhexis, intracapsular rings. In case of first-degree subluxation, retractors were fixed in the corresponding quadrant. Capsule tenses ring was implanted into capsular bag before implantation of flexible posterior chamber IOL. Postoperative period proceeded without complications. There was no evidence of inflammatory reaction. Visual acuity at discharge in 53.4% was 0.6-0.7, 30% was 0.4-0.5, 16.6% was 0.2-0.3. In postoperative period, stable position of IOL was observed, it was possible to save the capsular bag.


Use of the capsular tenses ring is effective and low-traumatic method that allows to reduce the amount of complication, to implant the IOL into its most physiological position, to achieve good functional results in the surgery of complicated cataract.
