Multilayer amniotic membrane transplantation for treatment of necrotising herpes simplex stromal keratitis with corneal perforation

Abstract Details

Purpose:

Evaluation of the effectiveness of transplanting multilayer amniotic membrane for treatment of necrotising herpes simplex stromal keratitis with corneal perforation.

Setting:

Department of Ophthalmology, Haseki Training and Research Hospital, Istanbul, Turkey.

Methods:

Five cases of herpetic necrotising stromal keratitis with developed corneal perforation were referred to our clinics between March 2011 and June 2014. All cases were treated with multilayer amniotic membrane transplantation. During the postoperative period, systemic or topical corticosteroids were given in support to the systemic antiviral treatment. The time of anterior chamber reformation and the healing of the corneal ulcer were evaluated. All cases were followed up for 3 -36 (mean 15,8±13) months.

Results:

In all the cases, the anterior chamber was formed on the first postoperative day and leakage had stopped. The superficial amniotic membrane patches disappeared or were removed within 10 to 19 days. In all cases, healing of the ulceration was complete in 15 to 30 days. In two of the cases , while the ulcer was healing, the deep graft fused with the corneal stroma and the surface was epithelised. The prophylactic doses of systemic antivirals was continued for 3 to 24 months in all cases and herpetic keratitis recurrence was not observed during follow up.

Conclusions:

Multilayer amniotic membrane transplantation is an effective treatment for herpetic necrotising stromal keratitis with corneal perforation, achieving the stopping of corneal leakage and stimulating the corneal ulcers healing.
