One year results of combined topography guided photorefractive keratectomy combined with corneal collagen cross-linking in mild keratoconus

Abstract Details

Purpose:

To evaluate the visual outcome of combined topography-guided PRK and collagen crosslinking in cases with mild keratoconus

Setting:

Venue: Eye care vision correction center, Cairo, Egypt

Methods:

In this single center, prospective interventional case series, 20 eyes of 12 patients diagnosed with keratoconus underwent topography-guided PRK using the CATz software (The Quest laser platform, NIDEK Co Ltd, Japan) utilizing the OPD topography system (NIDEK Co Ltd,Japan) .The maximum ablation depth was set to 70 microns and the optical zone was set to 5.5 mm with a transitional zone 1.5mm. This was followed by 30 minutes of 3Mw /cm 2 collagen cross-linking using OptoXlink cxl device(OptoGlobal, Australia) UCVA,BCVA and corneal topography were assessed at 1 ,6 and 12 months Follow up period: One year

Results:

Mean preoperative UCVA was 1.39±0.5 LogMAR ,improved at 6 months to 0.36± 0.46 and at 12 months to 0.27± 0.4 LogMAR.(p<0.001) Mean preoperative BCVA was 0.27± 0.31, improving significantly at 6 months to 0.12± 0.19 and reaching 0.08± 0.18 logMAR.at 12th months (p<0.03) 20% of eyes had no improvement in BCVA , 50% gained one line, 10% gained two lines, 10% gained 3 lines and one eye (5%) gained 4 lines . 1 eye (5%) lost one line in BCVA at 12th months. Mean preoperative cylindrical error was 2.13 D which improved to 1.13 D at 12th months.

Conclusions:

Combined topography –guided PRK and corneal collagen cross-linking is associated with statistically significant improvement of UCVA and BCVA.
