Femto-Laser assisted Anterior Lamellar keratoplasty using VisionGraft

Abstract Details

Purpose:

Femto-laser is a new and evolving technology with high potential fields in Ocular surgery. The dissection of the deep stromal layers involves a successful lamellar keratoplasty without interface haze.

Setting:

Al-Azhar university & Nour El Hayat Eye Center

Methods:

In 13 eyes of 11 keratoconus patients, deep stromal dissection up to pre-Descemt’s layer is achieved using a Femto-Laser (Victus, Bausch & Lomb) performing a surgical plane of 95% of stromal thickness leaving a smooth residual corneal stroma of 8 to 15 microns. A VisionGraft (TBI, USA) Gamma Irradiated donor cornea is then sutured to the recipient.

Results:

No operative complication was recorded all through the study. All patients had a minimum follow-up of 13 moths. The mean Preoperative best corrected visual acuity (BCVA) is 20/200 (20/150 to 20/400). Postoperative wound gap was eminent 6 days following surgery and a re-suturing was applied. Also recorded; a case of resistant re-epithelization for more than 2 weeks which was treated by bandage contact lens and autologous serum. No clinically significant interface was appreciated during the postoperative period.

Conclusions:

Combining both cutting edge of technology Femto-laser and VisionGraft corneal donor; achieves a safe and reproducible anterior lamellar keratoplasty. Long term follow-up should be considered in further studies in order to assess the safety of such new technique.
