Abstract Details

Purpose:

To show the numerical indices derived from curvature, BFS and BFTA elevation, pachymetry and WF that presently allow an early diagnosis of keratoconus and improved our preoperative evaluation of patients going to laser refractive surgery.

Setting:

BOS-Eye Bank of Sorocaba, Sao Paulo, Brazil and Private Eye Clinic, Sousas, Campinas, Sao Paulo, Brazil

Methods:

Case Series of normal corneas that successfully went to refractive surgery, corneas with suspected initial keratoconus that were rejected for surgery and corneas with established diagnosis of keratoconus.

Results:

Many indices are presently available that allow an early diagnosis of risk factors of post-operative ectasia and keratoconus such as the aspheric asymmetry index (AAI), the thickness progression index (TPI), the cone location and magnitude index (CLMI), keratoconus prediction index (KPI), vertical total corneal coma or percentage thickness ablation (PTA). The regular use of these indices in a logical diagnostic decision tree strategy improved our preoperative evaluation of patients wishing refractive surgery. Despite keratoconus is a disease traditionally defined as ectasia of the anterior corneal surface, earliest signs seem to be more related with the asymmetry of the posterior corneal surface.
