Results of cataract facoemulsification after ahmed valve drainage implantation

Abstract Details

Purpose:

To study the effectiveness and frequency of cataract phacoemulsification complications after implantation of Ahmed drainage.

Setting:

Ufa Eye Research Institute, Ufa, Russia

Methods:

Phacoemulsification of complicated cataract was performed in 25 patients in 3-36 months after previous surgery for refractory glaucoma with implantation of Ahmed drainage. By the time of phacoemulsification IOP in all patients equaled 22-29 mm Hg, corrected visual acuity - 0.02 - 0.08. In order to reduce IOP Ahmed valve drainage was rinsed with saline via 26G cannula.

Results:

The main tunnel incision was shifted by 2-3 mm into the nasal or temporal side from the tube of the drainage to prevent obturation of the drainage tube by lens masses. After surgery, visual acuity improved to 0.07 in 6 eyes, 0.2 - 4 eyes. IOP decrease by 2-5 mm Hg from the initial level was observed in all patients. In all cases IOP in the early postoperative period did not exceed 22 mm Hg. At later times (from 6 months to 1 year) hypertension was observed in 3 patients. It was compensated by the appointment of local antihypertensive therapy.

Conclusions:

Cataract phacoemulsification in patients with refractory glaucoma after implantation of Ahmed valve drainage contributes to improvement of visual function with simultaneous compensation of intraocular pressure.
