Surgical results of 23G pars plana vitrectomy combined with phacoemulsification and intraocular lens implantation

Abstract Details

Purpose:

To present the results of phacovitrectomy and intraocular lens implantation in eyes with vitreoretinal pathology with emphasis on the anterior segment complications.

Setting:

Şişli Hamidiye Etfal Education and Research Hospital, Department of Ophthalmology, İstanbul, TURKEY.

Methods:

This retrospective, single center study included 91 eyes of 80 consecutive patients undergoing transconjunctival 23 G phacovitrectomy and hydrophobic intraocular lens (IOL) implantation in a single center during a 3-year period with a minimum 2 months follow-up.(mean 14 months) Baseline and last visual acuity (VA), age, diagnosis, presence of systemic disease, preoperative lens status, history of any therapeutic intervention prior to surgery, type of intravitreal tamponade, types of complications, need for additional intervention and the latest ocular examination results were considered.

Results:

Mean age was 65 years (29-85 years) DM (74%), HT (56%), DM+HT (45%) and coronary artery disease (18.7%) were the accompanying systemic diseases.Diagnosis included macular surface disease (52%), vitreus hemorrhage(VH) (31%), tractional retinal detachment(RD)-diabetic retinopathy-PVR (11%) and rhegmatogenous RD (5.5%). Mean preoperative VA was 0.10±0.14(0.001-0.5). VA increased in 61%, remained stable in 24% and decreased in 15% of the eyes. Mean latest VA was 0.22±0.22 (0-0.9). VA change was significant.(p 0.001)Complications related to IOL including posterior capsule opacification-IOL decentralization-pupil capture were seen in 15 eyes.Eyes with silicone oil tamponade had lower VA than eyes with gas tamponade.(p 0.001)

Conclusions:

Phacovitrectomy with IOL implantation is a safe and effective surgery providing good visualization during vitrectomy and preventing a cataract operation in another session in such a patient group with serious systemic diseases.
