The effect of uncomplicated cataract surgery on macular and choroidal thicknesses

Abstract Details

Purpose:

The aim of this study was to evaluate the changes of macular and choroidal thickness after uncomplicated cataract surgery with phacoemulsification, using enhanced depth imaging spectral domain optical coherence tomography (EDI-OCT).

Setting:

Department of Ophthalmology, Hospital de Santa Maria (Centro Hospitalar Lisboa Norte), Lisbon, Portugal. No financial disclosure of any of the authors.

Methods:

In 15eyes of 15patients, the macular retinal (RT) and choroidal (CT) thicknesses were measured preoperatively and at 1week and 1month after phacoemulsification using EDI-OCT Spectralis(Heidelberg,Germany). Measurements were performed at the fovea and at 750, 1500 and 2250 ìm from the fovea, in the horizontal and vertical scans. The mean of results obtained in each OCT scan was calculated. The RT and CT at 1 week and 1 month after phacoemulsification were compared to the respective values in the preoperative period(PP). Full ophthalmological exam was performed in each visit. Axial length, spherical equivalent and intraocular pressure were recorded.

Results:

Mean-subfoveal CT(SFCT) measured at PP,1week and 1month were 195,93±53,6µm; 266,3±82,3µm and 234,7±56,4µm,respectively. Mean central RT at PP,1 week and 1 month were 243±24,9µm; 254,7±27,4 µm and 272,9±60,3µm,respectively. There was a statistically significant increase in SFCT at 1week(p=0,03) and 1month(p=0,021).Mean CT in horizontal scans(HS) increased to 228,15±63,4µm(p=0,019)at 1week,and to 226,7±59,9µm(p=0,013)at 1month.In vertical scans(VS), CT increased to 241,8±70,2µm(p=0,007) at 1 week, and to 235±64,7µm(p=0,004)at 1 month. Mean RT in HS and VS at 1month increased to 321,9±16,6µm(p=0,018) and 318,9±19,4µm(p=0,047),respectively.

Conclusions:

Uncomplicated phacoemulsification can induce nonpathologic increases in choroidal thickness. The macular retinal thickness at 1 month can also increase sub clinically due to the inflammatory insult caused by phacoemulsification. The macular OCT is useful in monitoring the subclinical macular retinal and choroidal changes in thickness that can occur after cataract surgery.
