Measurement of lens density using Scheimpflug imaging system and LOCS III system after intravitreal dexamethasone implantation

Abstract Details

Purpose:

To assess the cataractogenic effect of intravitreal dexamethasone implant using the Lens Opacity Classification System III (LOCS III) and Scheimpflug quantitative images.

Setting:

Marmara University Faculty of Medicine, Department of Ophthalmology

Methods:

This study included eleven eyes. The objective average lens density was measured using the Scheimpflug system while subjective grading was determined using the LOCS III in eyes that treated with slow releasing dexamethasone implant for macular edema associated with branch retinal vein occlusion. The crystalline lenses were graded at baseline as well as 4, 8 and 12 weeks after implantation.

Results:

The mean (±standard deviation) LOCS III grading scores for nuclear opalescence was 1.27±0.46 at baseline, 1.81±0.40 at 4 weeks, 1.80±0.42 at 8 weeks and 1.80±0.42 at 12 weeks. The mean Scheimpflug-measured average lens density was 9.27±1.60 at baseline, 11.22±1.74 at 4 weeks, 10.04±1.80 at 8 weeks and 10.06±1.12 at 12 weeks. The mean Scheimpflug-measured maximum lens density was 14.32±4.83 before injection, 15.04±2.92 at 4 weeks, 15.38±3.60 at 8 weeks and 16.65±5.04 at 12 weeks. Compared to baseline, the mean LOCS III grading scores and the mean Scheimpflug-measured average lens density were significantly increased at 4 weeks (p=0.014 and p=0.023, respectively).

Conclusions:

Lens density measurements by Scheimpflug system as well as subjective LOCS III grading system confirmed cataractogenesis following single dexamethasone implantation beginning from the 4th week. This effect persisted through the study period. Cataractogenesis model appeared to follow intravitreal drug time-concentration curve.
