Purpose:

To demonstrate usefulness of Tocular for correct intraoperative alignment of toric ICLs.

Setting:

Sarwate Eye Clinic & Laser Centre, Satara, India- A tertiary eye care centre.

Methods:

Tocular is an ocular device which can be mounted on any Mollar Wedel Microscope eye piece. It has manually rotatable knob with 180 degrees possible rotation. It has angular scale with small reticules in 5, medium reticules in 10, 2 large reticules at 0 and 180 degrees position. Preoperatively-limbus was marked at 3, 6 & 9 o clock position on slit lamp with patient in sitting position. After toric ICL implantation knob was rotated to align the the reticule scale markings with the markings at limbus & then it was rotated until it’s axis aligned perfectly with the axis on the scale.

Results:

Postoperatively in all cases, there was no residual astigmatism indicating a perfect alignment of toric ICLs.

Conclusions:

Tocular is more convenient tool for perfect positioning of toric ICLs due to coaxial alignment of it’s scale with eye during surgery.
