In-vitro analysis of optical quality in monofocal, bifocal and trifocal intraocular lenses of the same manufacturer

Comparison of optical quality in three different intraocular lens (IOL) designs.

In a laboratory study, optical bench analyses were performed with the OptiSpheric IOL Pro (Trioptics, Germany). We compared the modulation transfer function (MTF), MTF area (MTFA) measurements and through focus scans (TFS) of three different IOL models based on the same platform: CT Asphina 409M (monofocal), AT Lisa 809M (bifocal) and AT Lisa Tri 839M (trifocal) from Carl Zeiss Meditech/Germany. For each lens model 10 repeated measurements were performed in 5 IOLs with a dioptric power of +21.0 D. The United States Air Force (USAF) target picture was used to document the image quality.


TFS analyses showed a single far peak for the monofocal, a far and a near peak for the bifocal and a near, intermediate and far peak for the trifocal IOL. Mean MTF values (50 lp/mm far focus) were 0.804 for the monofocal, 0.448 for the bifocal and 0.387 for the trifocal IOL. The USAF analyses showed the best image quality at intermediate distance with the trifocal IOL.


The monofocal IOL (CT Asphina 409M) showed the best image quality for a single focal point with the highest MTF values. The bifocal IOL (AT Lisa 809M) offered the best near image quality. For intermediate distance the trifocal IOL (AT Lisa Tri 839M) achieved higher MTF values which was confirmed in USAF target analysis.
