Filamentous fungi keratitis in contact lens wearers

Abstract Details

Purpose:

To analyze the clinical cases of filamentous fungi keratitis and to draw attention to the right and rapid diagnosis as well as to the complex treatment and diverse results of treatment.

Setting:

The study was conducted at the Ophthalmology Clinic of Riga East Clinical University Hospital in year 2014.

Methods:

In year 2014, 4 cases of filamentous fungi keratitis was diagnosed. All the cases were laboratory approved. Fusarium keratitis was diagnosed in 2 cases and in other 2 cases the Acremonium falciforme and Aspergillus fumigatus was proven. Mean patients age was 35 years, all were contact lens wearers. The first complains in all cases were non-specific- lacrimation, sensation of sap and blurred vision. At the begining patients were treated by local anti-inflammatory drops and corneal gel. The specific antifungal ex-juvantibus treatment was started after 9-11 days. Later the selective treatment was added.

Results:

Recovery despite using selective medicamentous treatment was not reached in none of the cases. In 2 cases corneal transplantation was done (in one case in both eyes). One patient had evisceration with following silicone implantation. One more patient refused the corneal transplantation and continues medicamentous treatment.

Conclusions:

Initial misdiagnosis is common because of variable and non-specific symptoms. Specific ex-juvantibus treatment with following selective treatment should be started as soon as possible to improve number of recovery. Fungal keratitis is serious disease with life quality threating prognosis.
