Intrastromal corneal ring segment implantation in eyes with third stage progressive keratoconus

Abstract Details

Purpose:

Keratoconus is a bilateral , degenerative, ectatic corneal disease with central or paracentral corneal thinning. The reported incidence ranges from 1.3 to 25 per 100,000 per year across different population. Advanced treatment modalities such as newer contact lens designs, collagen crosslinking, intracorneal ring segments, photorefractive keratectomy, and phakic intraocular lenses occurred these years.For eyes with contact lenses and glasses intolerance and keratoconus progression -implantation of intrastromal corneal ring segments (ICRS) might be the basic step in the treatment.Our goal was to reveal effectiveness of ICRS in patients with progressive III stage keratoconus.

Setting:

Yerevan State Medical University, Medical Center, Armenia. After M.Heratsi

Methods:

The surgery was done on 58 eyes with severe III stage progressive keratoconus. Patients underwent ICRS (Keraring) implantation with femtosecond laser (Ziemer CristalLine).We did not have any kind of complications during and after the surgery. Patients was follow up for at least six months. All patients were divided into 2 groups, first group:before the ICRS implantation and the second group:the same patients after the surgery.

Results:

In all the patients we have registered both; visual rehabilitation and absence of keratoconus progression. Mean corneal radius range before and after the surgery was 6,9±0,09 / 7,21 ± 0,09. Corneal power before and after the surgery was 49,28 ± 0,7 / 47,1 ± 0,67. Mean UCVA before and after the surgery was 0,15 ± 0,02 / 0,41 ± 0,04.

Conclusions:

Implantation of intrastromal ring segments has two effects ; visual recovery and stop progression of keratoconus in eyes with keratoconus III stage. Using a Keraring tipe ICRS is safe and the results are very predictable in patients with third stage keratoconus. Using of a femtosecond laser for creating the tunnels is more predictable because it makes the surgery easier for patient with keratoconus III stage .
