Endothelial keratoplasty for the treatment of endothelial dysfunction associated with anterior corneal opacity

To evaluate the outcomes of Descemet stripping automated endothelial keratoplasty (DSAEK) for the treatment of endothelial cell dysfunction associated preoperatively with anterior stromal opacity.

Five patients with longstanding corneal endothelial dysfunction associated with anterior stromal opacification with a maximum depth of 150┬Ám underwent DSAEK. Such patients would normally be listed for penetrating keratoplasty. Patients were evaluated with complete ocular exam and Visante anterior segment optic coherence tomography (AS-OCT) pre and post operatively.


All the patients had significant improvement of best corrected visual acuity (BCVA) and decrease in intensity and depth of stromal opacification 3 months after surgery. One patient had complete resolution of opacification and 2 had BCVA of 20/30. One patient had restricted vision possibly related to DSAEK interface irregularity and one patient remained with significant corneal haze with phototherapeutic keratectomy indication.


Endothelial keratoplasty might be an alternative therapeutic procedure to penetrating keratoplasty in cases of endothelial dysfunction associated to anterior stromal opacities.
