To describe a new procedure to improve surgical maneuvers and to check the graft location and orientation performing endothelial keratoplasty with Lumera microscope and OCT combined image during the surgery.

We present our experience performing descemet membrane endothelial keratoplasty (DMEK) and descemet stripping automated endothelial keratoplasty (DSAEK) with a technique aided by OCT (Lumera Rescan 700 Zeiss Medical Solution Surgical Microscope). This device provides a superior quality image during the surgery with the possibility of checking the graft with an OCT technology at any moment of the surgical procedure.


The introduction of OCT technology in endothelial surgery is a great asset to the success of the technique. It´s possible to check the situation of the graft into the anterior chamber and also the orientation, that is crucial with descemet roll in DMEK. Maneuvers such a corneal massage in DSAEK can also be observed in vivo with high image quality.You can also explore the edge of the descematorrhexis and the limits of the peripheral graft. With this technique the complications associated with the orientation and positioning of the graft are minimized.


OCT guided posterior lamellar keratoplasty suppose a great step for the cornea surgeon. It allows to check the proper descemet graft unfolded and also its orientation and position and therefore reduces surgical time and complications.
