Unilateral congenital cataract in a child with blepharophimosis ptosis epicanthus syndrome (BPES)

To report a case of unilateral congenital cataract in a child with Blepharophimosis Syndrome (BPES).


Narayana Nethralaya Superspeciality Hospital and Post Graduate Institiue of Ophthalmology, Bangalore, India


A single case report of one year old girl with BPES who presented with white pupillary reflex in the left eye. Examination revealed unilateral cataract in the left eye. This unique case and its management are discussed.


Surgical management of cataract in this case was technically more challenging due to the following reasons: 1. limited surgical space because of short horizontal palpebral fissure and epicanthal folds. 2. Leathery, partially absorbed membranous cataract which had to be removed with a modified technique.


BPES can be associated with many ophthalmic and facial abnormalities. To the best of our knowledge this is the first reported case of BPES with unilateral congenital cataract. Surgical management of cataract is more challenging in case of a blepharophimosis syndrome.
