Ophthalmologic repercussion of hemodialysis

The objective of this paper is to present ocular events that come as a consequence of hemodialysis

Clinical case report. A 65-year-old man complained from recurrent left ocular pain and amaurosis fugax during hemodialysis, associated with presentation after a posterior capsular rupture during phacoemulsification on the left eye. The past medical history included transperitoneal dialysis 4 years before and systemic hypertension.


Pre-dialysis exam showed: Best-corrected visual acuity (BCVA) of 20/40 in right eye (RE) and 20/100 in left eye (LE), with intraocular pressure of 12 and 8 mmHg respectively. Post-dialysis exam revealed: BCVA of perception of light at the left eye, inflammatory cells (Grade 1+) in the anterior chamber and 36 mmHg in LE. A slightly pale left optic nerve could be seen.


Hemodialysis may be a cause of a recurrent painful intraocular pressure spikes in a patient with unilateral anterior uveitis. Systemic hypotension at the beginning of the hemodialysis associated with a left carotid stenosis of 30-50% might produce a posterior ocular ischemic event.
