Prior intralenticular vascular endothelial growth factor inhibitor injection complicating cataract surgery

Abstract Details

Purpose:

To document the signs associated with intralenticular vascular endothelial growth factor (anti-VGEF) inhibitor injection, the intraoperative challenges during cataract surgery and the surgical techniques adopted to achieve a clinically acceptable result.

Setting:

Tertiary Teaching Eye Care Institute, India

Methods:

Two cases with prior intralenticular vascular endothelial growth factor inhibitor injection without recorded iatrogenic lens damage underwent cataract surgery. Clinical signs included visible anti-VGEF delineating the lens matter and on the posterior capsule. Both cases had iatrogenic preexisting posterior capsule defects.

Results:

The iatrogenic preexisting posterior capsule defects could be well managed by a combination of slowmotion phacoemulsification, automated anterior vitrectomy and multipiece IOL implantation in the sulcus with optic capture by the rhexis.

Conclusions:

A high index of suspicion in eyes that have a prior history of intravitreal injections and management of complications by appropriate surgical techniques, viscoelastic devices, automated anterior vitrectomy and multipiece IOL implantation in the sulcus with optic capture by the rhexis can give good clinical outcomes inthese challenging cases.
