Abstract Details

Purpose:

Glistening and whitening (sub-surface nano glistening) are caused at intraocular lens (IOL) optics as a complication after cataract surgery. These are related with water phase separation and there are some cases who decreased visual function. In this study, the degree of glistening and whitening were classified using slit lamp.

Setting:

Department of Ophthalmology. Dokkyo Medical University, Tochigi, Japan.

Methods:

Fourteen hydrophobic acrylic intraocular lenses (AcrySof, Alcon) were extracted because of dislocation or visual disturbance. The lens capsule and tissue attached to the extracted IOLs in physiological saline were removed, and the IOLs were placed into artificial model eye. The glistening and whitening were observed using slit lamp and classified into 0 (none) to 4 (severe). Next, light transmission of the IOL in physiological saline was measured using a spectrophotometer (U-4100, Hitachi). After that a correlation coefficient between glistening, whitening and time periods after IOL implantations were calculated.

Results:

Although grade of glistening does not correlate to the periods after IOL implant; however, grade of whitening was increased with the periods after IOL implant (correlation coefficient = 0.61). When the grade of glistening and whitening increases, light transmittance is reduced. The correlation coefficient between grades of glistening and light transmittance is -0.60 and whitening is -0.46.

Conclusions:

Classification of glistening and whitening using slit lamp is useful to evaluate the decrease of light transmission. It will be useful information to decide the indication of IOL extraction.
