Femtosecond-assisted cataract surgery vs conventional phacoemulsification: comparative endothelial cell density and morphology analysis

Abstract Details

Purpose:

Study, describe and compare endothelial cell loss and morphological changes after femtosecond assisted cataract surgery (FLACS) and conventional phacoemulsification cataract surgery.

Setting:

Hospital Universitario Donostia ,Gipuzkoa,Spain

Methods:

100 patients with bilateral cataract were included for surgery, receiving FLACS in one eye and conventional phacoemulsification in the fellow eye. Surgeries were performed with VICTUS Femtosecond Platform(Bausch+Lomb) and Infiniti Vision System(Alcon) followed by Envista(Bausch+Lomb) IOL implantation. A complete ophthalmological examination including endothelial analysis was performed before, 1-day, 1-week, 1-month, 3-months and 6-months after surgery. For a set area, total counted cell number, average(±standard deviation(SD)) cell size(μm2), cell density(cells/mm2) and cell hexagonality(%) are studied. This trial counts with Local Ethics Committee approval and patient informed consent was obtained before inclusion. Statistical analysis was performed with STATA.

Results:

In the FLACS group, mean(±SD) preoperative values for total counted number of cells, average cell size, cell density and cell hexagonality were 150,81±26,76, 430,42±75,93, 2351,84±401,48 and 54,6±8,36 respectively.6-month mean(±SD) values were 135,14±32,34, 484,93±112,34, 2160,96±449,28 and 54,76±8,15 respectively. In the phacoemulsification group, mean(±SD) preoperative values for total counted number of cells, average cell size, cell density and cell hexagonality were 150,71±27,58, 434,17±78,51, 2361,95±402,70 and 56,79±8,66. 6-month mean values(±SD) were 136,41±29,43, 481,30±103,84, 2163,20±415,33 and 56,24±8,89 respectively. Postoperative differences in cell number or changes in cell size, cell density or cell hexagonality between FLACS and phacoemulsification groups were not statistically significant (>0.05).

Conclusions:

In this series, there are no statistically significant differences in endothelial cell density or cell morphology when comparing the conventional phacoemulsification group with the FLACS groups in any of the visits.
