Clinical experience of small incision lenticule extraction in patients with high astigmatism more than 3 diopters

Abstract Details

Purpose:

To report clinical experience including visual outcome and complications of small incision lenticule extraction to correct astigmatism or myopic astigmatism more than 3D eyes.

Setting:

Nunemiso Eye Clinic, Seoul, Korea.

Methods:

64 eyes of 47 patients underwent small incision lenticule extraction(SMILE) to correct their astigmatism or myopic astigmatism and were followed up more than 3 months. 29 patients were men and 18 were women. Mean age of the all patients was 29.3 years old and the range of corneal thickness was 482~661microns. We made 6.0~6.5mm diameter lenticule and it was extracted through 2.0~4.0mm side-cut incision by using forceps. Estimated postoperative corneal thickness including 100~120micron cap thickness was at least 400microns. We checked uncorrected distance visual acuity(UDVA), manifest refraction and ocular condition after 1day, 1week, and 3months postoperatively.

Results:

Preoperative mean astigmatism was 3.37±0.48D (3.00 ~ 4.25D) and preoperative mean spherical equivalent was -5.52±1.66D (-2.00 ~ -8.19D). Postoperative mean UDVA was 0.94±0.21(POD 1day), 1.02±0.22(POD 1week), 1.03±0.22(POD 1months), 1.04±0.20 (POD 3months). 73% achieved UDVA 1.0 or better on the last follow up. Postop. mean spherical equivalent was -0.25±0.51D postop. mean astigmatism was -1.09±0.45D. 82% had a refractive outcome between ±0.5D of attempted refraction and 92% between ±1.0D. Suction loss occurred in one eye. There was no serious postoperative complication like keratectasia, corneal opacity.

Conclusions:

Small incision lenticule extraction is effective and safe vision correction procedure in treating high astigmatism more than 3 diopters. Visual recovery was quick and there was no vision threatening postoperative complication.
