Abstract Details

Purpose:

To describe our findings in the eyes of dissatisfied RLE customers receiving the Oculentis MplusX multifocal lens and how we have solved the problems.

Setting:

The Danish branch of the Memira clinics, Denmark( Part of a large chain of private refractive clinics in the Nordic countries)

Methods:

During 2013 – 2014 a total of 73 lenses (in 38 patients) of a novel multifocal asymmetrical design were implanted in our clinics. Seventeen patients (45%) complained of impaired distance vision resulting from double vision or myopic shift. These phenomena were investigated by photo analysis using superimposition of dilated slit lamp photographs and pupillometry images.

Results:

Spontaneous improvement occurred in one eye. One patient was lost to follow up. Rotation of the implant was performed in 26 eyes, relieving the symptoms in 50% of cases. IOL exchange for a monofocal implant was performed in 8 eyes. Twelve eyes required laser vision correction. All patients came out with good distance vision without loss of lines, half of them requiring reading glasses.

Conclusions:

In order to improve reading ability the manufacturer created a new asymmetrical design based on the original Mplus IOL. However, the enlarged near vision zone of the MplusX implant requires special attention to centration and rotation of the lens relative to the visual axis. Unknowingly neglecting this precaution resulted in a high proportion of dissatisfied customers with visual disturbances. Follow up and re-operation of these cases was very demanding and time consuming.
